Registration form instructions:

2. Print each form.
3. For each individual attending, complete FORM 1 (Registration and class selection).
4. For each family attending, complete FORM 2 (Field trips, Meals, Teen Program)
5. For each family attending, enter on FORM 3 (payment due) totals from FORM 1 and FORM 2.
6. For each individual attending, complete FORM 4 (liability release).
